How Tremors Affect Quality of Life in Parkinson’s Patients with Joon Faii Ong

Dr. Joon Faii Ong is one of the world’s foremost authorities on essential tremors. He has published more than 100 scientific papers in journals around the world and is well known for his contributions to understanding what causes tremor, how it can be treated, and its impact on quality of life.

In this article, Dr. Ong discusses how tremors affect the quality of life in Parkinson’s patients.

Parkinson’s Disease is a “non,” “familial,” or “sporadic” degenerative disorder of the central nervous system.

It results from a deficiency or loss of nerve cells that produce the neurotransmitter dopamine (DA) in areas of the brain such as the Substantia Nigra. The presence of Lewy bodies and cytoplasmic inclusion in neurons and glial cells is pathognomonic for this disorder.

Parkinson’s manifests itself in four main ways: tremor, rigidity, bradykinesia, and postural instability.

Parkinson’s is a progressive disorder. It begins with a very subtle tremor, which the patient or others around them may not even notice. As the disease progresses, rigidity and bradykinesia become more dominant, coupled with postural instability that often makes patients fall without warning. Eventually, the induced loss of DA leads to depression and dementia.

How Tremors Affect Quality of Life in Parkinson’s Patients

Tremor is the most common motor symptom that occurs in Parkinson’s patients. It can occur at rest or during voluntary movement, but it does not equally affect all Parkinson’s patients. In addition, the severity of tremor in a Parkinson’s patient can vary from barely noticeable to disabling.

An Essential Tremor (ET) is a type of tremor that occurs when one or more body parts move and shake involuntarily and uncontrollably. ET has many different characteristics and degrees (severity). Some individuals exhibit Parkinsonian symptoms such as stiffness, bradykinesia, and a resting tremor.

In Parkinson’s, ET usually develops as the disease progresses. In contrast to Parkinsonian tremor, which occurs at rest and is temporarily relieved by movement, essential tremor persists during movement or maintenance of a posture. The tremors associated with ET can be quite disabling for those who have trouble speaking, walking, eating, and writing.

In Parkinson’s patients with ET, the tremors can cause significant stress on a person’s quality of life. In fact, in one study of Parkinsonian patients with tremors at rest, those who had ET experienced more severe disability scores than those who did not have tremors at rest.

One of the goals of Parkinson’s medication is to reduce tremors. Parkinson’s medications either block the dopamine receptors to increase the amount of dopamine available in the brain or make it more difficult for neurons to release dopamine by other mechanisms. In other words, Parkinson’s medications “replace” the lost neurotransmitter.

In Parkinson’s patients with ET, there are many Parkinson’s medications that can help reduce tremors. However, Parkinson’s medications do not affect ET directly; rather, they act on the tremulous abnormal oscillatory discharge patterns in the brain to exert their effects on movement and tremor. In addition, some Parkinson’s medications also improve rigidity, bradykinesia, and postural instability in some Parkinsonian patients with ET. Helping Adults Heal from Past Abuse – Robert Turner

Adults who suffer abuse as a child or are raised in dysfunctional or abusive families share many symptoms. By the time they reach adulthood, most adults have buried their symptoms because no one was willing to listen. Adults need help recognizing symptoms and healing them because symptoms often cause depression, anger outbursts, self-destructive behaviors, or codependent relationships. According to Robert Turner, recognizing symptoms of abuse is the first step to recovery.

All abuse, including emotional abuse, is a crime against a child’s humanity. The perpetrator of the abuse instills fear and shame in the victim through threats and actual violence or by withholding love and support. The emotional scars from this type of childhood torture often affect every aspect of a person’s life into adulthood.

When you were a child, did your parents:

  • Constantly tell you that you were stupid and make fun of everything you did?
  • Shame and humiliate you in front of the company or at the dinner table?
  • Call you fat and ugly or make fun of your looks?
  • Verbally abuse you by using cruel words that cut into your self-esteem?
  • Physically abuse you in any way? This includes shaking, pushing, slapping, pinching, or biting.
  • Isolate you from friends and family by yelling at them or threatening them?
  • Was he always angry with you so that even little things made him yell?
  • Did your father withhold love, support, or attention because you made him angry?
  • Did he criticize all of your friends?

If any of these questions ring true in your life, then you may have been abused. The abuse lived only one generation away from where you are now because the abuser was a child at one time. It is important to understand that all of these behaviors are learned over a lifetime by the perpetrator. This means the abuse could have stopped with you, but it did not.

These symptoms can be overcome through treatment, support, and hard work by you for years, if not decades to come. Counseling, family therapy, and peer groups are the best forms of treatment to resolve deep-rooted issues. Many people seek individual therapy or attend Alcoholics Anonymous or Al-Anon meetings because they need help with their life issues. If you did not grow up in an abusive household but live with someone abusing you, get out and find a support group for battered women and their children.

Dealing with Past Trauma

Adults who were traumatized in childhood often re-enact the trauma with other people. The old feelings, unresolved from when they were young, surface when triggered by a current event, such as a breakup or loss of a job. And when those feelings are stimulated, it can bring on a flashback that is similar to what your body experienced when you were young.

The abused adult carries the past abuse in his mind and body through intrusive thoughts, night terrors, “flashbacks” of painful memories, or physical sensations that feel like they are happening again. These feelings can be so intrusive that the person does not realize she is safe, except for these moments when she is triggered by something in her current life. It is very hard for the person to stop thinking about the painful past and get back to present reality when this happens.

How to Address the Rising Cost of Education – Victor Restis

Student loan debt is skyrocketing.

According to the nonprofit Project on Student Loan Debt, student loan debt has risen more than 165 percent in the past decade and is nearly $1 trillion. The average college graduate owes almost twice as much upon graduation compared with a decade ago. A large chunk of that money goes toward education expenses at for-profit colleges, which feed off the need of students to obtain a degree to land better jobs. For-profit colleges face federal scrutiny for allegedly exploiting students at their most vulnerable by luring them to courses with false promises and saddling them with tens of thousands of dollars in debt.

Freelance writer Victor Restis has some thoughts on how to address the rising cost of education. Not all college students take on student loan debt. However, since the early 2000s, the cost of college has been going up faster than inflation and faster than family incomes. Policymakers should address this problem as it creates a false sense of reality that one needs a degree to earn a living wage. It also creates an economic burden for those who do take on debt to obtain their degree. With fewer jobs available in today’s economy, it is unclear how debt-burdened graduates will repay the loans they take out for college.

What could be done to make higher education affordable again?

The solution lies not in finding ways for colleges to lower tuition but rather in helping students find other means to finance their education. There are several options given that student loans are not the only way to pay for one’s studies. For example, work-study programs could include paid internships. While there has been growth in these programs over the past decade, many of them remain small or underutilized by potential participants because of funding or bureaucratic barriers at the college level.

Another option is to use the student’s budget, i.e., a Pell grant, to attract private investment on campus once the student has been accepted on the sole basis of merit and ability. The government would increase funding for these grants, which would be partially bankrolled by investors who would see their money grow over time through simple interest payments, given that students do not stop attending class or flunk out. This is similar to how homeowner loans are funded — in this case, however, the borrower is the government rather than a private lender.

What about loan repayment?

A final proposal would involve changing the terms of existing federal loans to reduce monthly interest rates that can reach up to four percent per annum for recent graduates. The way to accomplish this would be to buy out existing loan holders at a fraction of the market value to clear the market. This would have the net effect of reducing interest rates for future classes of students, who would then afford more education options.

An important part of this final proposal hinges on the government’s ability to buy out existing student loan holders at a fraction of the market value to clear the market. This will allow future borrowers with debt obligations to lower their monthly payments by refinancing their loans at the new rate.

4 Common Causes Of Family Arguments

No family is perfect and familial relationships can be really difficult and complicated. All families have arguments and fights every so often, and some even get to the point where people who were once close become estranged from each other. In order to avoid this happening to you, you should be prepared for some of the types of arguments you may have with your family at some point. Here are 4 common causes of disputes and disagreements within families.

Estate Planning

Oftentimes when the matriarch or patriarch of a family passes away, there can be nasty arguments among those left behind regarding the estate. In order to keep this from happening, many people opt to start estate planning early so that everything is clear, fair, and legal. Unfortunately, the planning process itself can also cause arguments. Make sure to protect your assets by having a lawyer present for all conversations you have with your children or other family members regarding your estate.

Illness

If someone in a family becomes gravely ill, everyone who loves that person will be under a lot of stress and therefore less equipped than usual to deal with things in a level-headed way. Normal conversations can become tense, and there can be major disagreements or fights over how best to care for the ill family member. Try to remember that you all have the ill person’s best interests at heart and in the end you all want the same things. This will help to keep you more calm during these hard times.

Children

When someone in your family has children, you may notice that everyone else in the family will often try to weigh in on the best ways to raise them. This can cause fights between family members when they ultimately disagree on decisions regarding the children. In these situations it is best to keep in mind that ultimately each parent (or legal guardian) gets to make the decisions when it comes to underage kids. Even though the Aunts, Uncles, Grandparents, etc. love the children very much, it is not their choice to decide where they go to school, what their bedtime is, and so on.

Significant Others

If you or someone else in your family is dating or married to someone that other family members do not like, this can be a huge cause of arguments. You may even be forced to choose between your significant other or your family, so it is a good idea to think very carefully about this before making any brash decisions. Listen to your family and consider what they have to say. At the end of the day, it is your choice who you decide to date or marry, but often your family is only acting out of love for you.

Family disputes are never fun and can be awkward, uncomfortable, and even devastating. Keep in mind that just because you are related to someone doesn’t mean you have to be tied to them forever, but that being said, do try to approach all arguments with a clear mind.

Different Types of Drill Bits

A drill is one of the most useful tools in any homeowner’s toolbox, and it’s required for almost every project, whether it’s a minor decor change, a significant room alteration, or a major building development. However, without suitable drill bits for the task, a decent drill is useless.

This article will assist you in determining the best single bit or drill bit set for your project.

What are Drill Bits Used For?

Drills are available in a variety of materials and sizes, allowing you to drill holes in anything from wood, metal, plastic, ceramic tile, porcelain and concrete. Steel, aluminium, copper, cast iron, sheet metal, fibreglass, brick, vinyl flooring, and other materials can also be drilled with drill bits.

Drills are available in a variety of shapes and sizes to enable a variety of applications. Which drill bit is appropriate for the task at hand? Keep reading to learn about the many different types of drill bits that you’re likely to encounter.

Drill Bit Function

The shank and chuck are the two most obvious parts of a drill when it comes to drills, although there are many more components that go into making one. The following is a quick rundown of what you need to know about how drills and drill bits function:

The shank is the portion of the drill bit that fits into the drill and is secured by the chuck. A round shank aids in the accurate centring of a bit in the chuck. Hexagonal flats on a hexagonally shaped shank make it easier for the chuck to grip the drill bit.

The shank of the bit is made out of steel. Steel-shaft bits are intended to be used with 1/4″ impact drivers and maybe exchanged quickly. SDS (slotted drive system) shanks are used for concrete drills and demolition bits, and they’re designed specifically for SDS-Plus, SDS Drill Bits or SDS-Max rot.

The chuck is the drill bit attachment component of the machine. The majority of home power drills have a 3/8-inch or 1/2-inch chuck. Larger chunks are available in 5/8-inch and 3/4-inch sizes, which are generally used on heavy-duty and industrial power drills and drill presses.

Twist Drill Bit

Twist drill bits are suitable for light metal, wood, plastic, metal, ceramic and masonry. With metal, wood or ceramics, it is useful for house repairs, maintenance and construction projects. Rotary drilling on a general level; the most popular drill bits for home use.

Brad and Pilot Point Bit

Brad-point drill bits can also be used on wood. Ideal for furniture making, cabinetry, and general woodworking. W-shaped centred point; clean exit hole is produced. For wood DIY projects, this is typically the ideal drill bit.

Auger Drill Bit

Auger drill bits are also used on wood. For bigger woodworking and construction tasks, they’re particularly useful. Screw-tip reduces the amount of force needed to operate the tool; less effort is required.

Forstner Drill Bit

Forstner drill bits may be utilized on wood. It’s useful for carpentry and construction projects because it makes clean holes with a flat base and works better in a drill press than a handheld drill.

Spade Drill Bit

Spade drill bits are also useful for drilling holes in wood. Frame holes, electrical, plumbing, and fine woodworking tasks are just a few examples of what you can accomplish with these bits. It bores large-diameter holes.

Do my grandparents have any rights to visitation by law in Alabama

Although there are limited rights to visitation of grandparents, there are certain conditions that can be met that will permit visitation. An experienced Huntsville divorce lawyer can help advise you on whether a grandparent can petition the court for visitation rights and be successful. The grandparent visitation statute is established in AL Code § 30-3-4.2 (2016), where it states that visitation of a grandparent can be granted if the grandparent can prove that not permitting it would cause harm to their grandchild.

Harm in this case is defined as “a finding by the court, by clear and convincing evidence, that without court-ordered visitation by the grandparent, the child’s emotional, mental, or physical well-being has been, could reasonably be, or would be jeopardized.” This can be a difficult standard to prove, but not impossible.

How AL Code § 30-3-4.2 (2016) Applies

As mentioned above, harm, as defined by AL Code § 30-3-4.2 is an important determinant for visitation rights. To that end, grandparents are able to file an action in court where their grandchild resides or they can file a motion to intervene with any state court before issues of custody arise in order to establish reasonable visitation rights. It is important to note that certain conditions apply:

1. An action for a divorce or legal separation has been filed by the parents, or the marriage has been severed by death or divorce.

2. A child has been born out of wedlock and the petitioner is a maternal grandparent of the child.

3. The child was born out of wedlock, the petitioner is a paternal grandparent of the child, and paternity has been legally established.

4. An action to terminate the parental rights of a parent or parents has been filed or the parental rights of a parent has been terminated by court order; provided, however, the right of the grandparent to seek visitation terminates if the court approves a petition for adoption by an adoptive parent, unless the visitation rights are allowed pursuant to Section 26-10A-30.

If visitation rights are challenged, or rebutted, the petitioning grandparent can argue that they have a significant and viable relationship with the child. To establish a significant and viable relationship to the court, they must demonstrate evidence of the following kind:

1. The child lived with the petitioning grandparent for at least six consecutive months either with or without a parent within the three years prior to filing the petition.

2. The child lived with the grandparent as a caretaker on a regular basis for at least six consecutive months within the three years prior to filing the petition.

3. The petitioning grandparent has had frequent or regular contact with the child for at least twelve consecutive months that resulted in a strong and meaningful relationship with the child within the three years prior to filing the petition.

Alabama law states that “as a matter of public policy, this section (AL Code § 30-3-4.2 (2016)) recognizes the importance of family and the fundamental rights of parents and children.” But by making a path for grandparents to have a continued role in their grandchildren’s lives, a glimmer of hope remains.

Flat Head Syndrome and Preventing It

As a parent, you want to ensure your newborn stays both safe and comfortable even during normal activities such as sleeping. One issue to be concerned with is called flat head syndrome or positional plagiocephaly. It is a common issue and occurs in 47% of newborns and is more common in children that are born prematurely. It is a condition where the side or the back of the baby’s head is flat and slightly misshaped. While it is a condition that a child grows out of as they get older and grow it can lead to neck issues requiring physical therapy in some cases.

The Causes of Flat Head Syndrome

Flat head syndrome is caused by the baby’s sleep positions as infants and newborns spend a great deal of time sleeping. A lack of motion and staying in the same sleeping position for an extended period of time is what causes the flattening of the skull. This can occur in cribs, car seats, swings, strollers, and baby carriers. Flat head syndrome is not hard to identify as in addition to the previously noted flattening there is also a lack of hair where the flattening occurs, and in some cases, the ears can appear pushed forward on the head.

Using a Pillow to Prevent Flat Head Syndrome

There are many ways to prevent flat head syndrome and one of the most effective is to use a supportive pillow. A Lil Cuddler baby pillow is designed to prevent flat head syndrome in infants offers several benefits.

  • Added Support: a supportive pillow spreads out the weight and pressure that can become focused on the head if your infant’s pillow lacks proper support. This increases comfort and also prevents aches and body pains.
  • Better Sleep: a supportive pillow also improves your infant’s sleep the added support, improved airflow, and increased comfort allows your child to rest easier. This improves the quality of life and can aid in development.
  • Prevent Health Problems: in addition to helping prevent flat head syndrome, a supportive pillow has other health benefits. It can prevent growth and development issues related to the nervous system by supporting the neck and spine.

Final Thoughts

As a parent, it’s normal to be concerned about your child’s safety and wellbeing. One of the best things you can do to keep your child safe is to take preventive steps to prevent health issues and concerns. A supportive pillow designed to keep your child safe can be a simple solution that prevents health issues such as flat head syndrome.

How to Get Toddlers into Cycling

Your childhood is incomplete if you do not know how to cycle. That may just be an opinion but it is a widely accepted opinion. It is in fact, an essential activity for the growth of your child. It helps your child improve their focus and physical health. What’s more, you can also improve your relationship with your child. When you teach your kid how to ride a bike, they feel close to you and gain trust.

However, your child may not be too fond of cycling, so here is what you can do:

How To Teach Your Child To Cycle

Make Them Feel Secure

Riding a cycle can be a scary experience for many children. Not all children are ready to take on a challenge and be unfazed by injuries. They may be scared at first, which is why you must do everything in your power to ensure that they are not. You can invest in a 12 inch 12 inch balance bike balance bike that is easy to ride and will give your child the right kind of balance. It does not have pedals, which allows your child to push the bike forward with their feet.

This helps maintain a steady balance and your child will not be too scared. Moreover, you can get them safety gear such as a helmet, knee pads and elbow pads. These items will make them feel more secure and ready for the risks. When teaching kids, do not go for a street with high traffic as your child may not have the confidence to ride with distractions which is why it is best to teach them in a secluded street.

Choose A Comfortable Time

Everything has an optimal time. You cannot expect your child to always be up and ready for a bike lesson. This is why it is the responsibility of the parent to decide on a time that is the best for their child. For instance, you mustn’t request your child to ride their bike on an empty stomach or while they are drowsy. They might not have the concentration they need and will not be able to ride the bike comfortably.

It would be best to choose a time that is a best fit for them and you and you shouldn’t hurry the learning process of riding a bike.

Do Not Have Unrealistic Expectations

Children can crumble under a lot of pressure. Bike riding is a fun activity,so do not make it excessively challenging for your child. They shouldn’t have to fear pleasing your expectations when they are learning to ride. It is your responsibility to ease them in to it and let them learn at their own pace. The more achievable and realistic expectations you have, the better your child will learn how to ride the bike.

In Conclusion

Learning to ride a bike is a beautiful part of childhood. Avoid making it into a harrowing memory for your kid. Also, it would be best to ensure that you take notice of their comfort and cater to it as best as you can.

CHOOSING CARE: RESIDENTIAL CARE SETTINGS EXPLAINED

WHAT IS RESIDENTIAL CARE?

Residential care refers to long-term care for people or children who live in a residential environment instead of their homes or families.

Along with Oakland Care, there are several choices for care homes in Eastbourne based on the individual’s needs. People with impairments, mental health issues, learning difficulties, Alzheimer’s disease, dementia, or those who have a fragile old age are commonly treated home with extra assistance from home care facilities through paid or voluntary carers, such as their family and friends.

RESIDENTIAL CARE SETTING:

Several older adults live in a residential care setting, generally in individual rooms, and on-site access care twenty-four hours daily. A facility of care homeclassified merely as a home for care typically offers personal support and care, such as assistance with washing, clothing, and drug treatment. Additional support may be provided, for example, to help eat food, to promote agile mobility, etc. Some nursing facilities are registered for a particular health need, such as dementia or terminal illness/end of life.

These are some examples (and by no means a complete list) of help for residential/personal care that might be requested or expected:

MORNING TIME SETTING:

  • Assisting in waking up and getting ready for the day
  • Showering or bathing
  • Application of lotions and creams as necessary
  • Hygiene oral (brushing teeth, denture care, etc.)
  • Applying cosmetics and assisting with hairstyle
  • Assistance with shaving
  • Foot hygiene
  • Dressing

DAY TIME:

  • Help to ambulate about the house and its environs
  • Toilet help, including with a cabinet or bedpan
  • Assistance during meals when necessary
  • Ensure people have the chance to connect socially with others (as per their wishes)
  • Make sure residents are properly moisturized and use the required fluids

NIGHTTIME:

  • Preparation for bed
  • Reposition in bed to extend and avoid bed damage
  • Change of continent pads and cleaning of intimate regions
  • Change or maintain stoma or catheter bags or other healthcare procedure

Care homefacilities are community-based care settings for those who cannot live independently. Residential care arrangements licensed by countries with varied definitions include small-scale houses and big assisted living complexes, and most often have personal care.

HOW DOES RESIDENTIAL CARE SETTING WORKS:

Residents can remain in residential care facilities for long-term, short-term relief and emergencies. People struggling with everyday life because of their age, physical impairment, learning disabilities, mental health problems, addictions, or other medical needs have a residential home. A house can be accessible to persons with various care requirements or specialized, assisted by care assistants, in physical impairment or dementia situations. Depending on the stage at which they are located, persons with dementia can live in a residential environment as they may require care and aid in everyday activities, but they still do not need health care.

CHOOSING RESIDENTIAL CARE:

Choosing a long-term care facility may appear to be a difficult task. Still, by asking the right questions and keeping an eye out for specific qualities, the process can be more manageable and successful. Before choosing residential care for a person with dementia, it is important to understand the individual’s needs because there are many settings in which it may be provided.

Residential care setting selection may be a difficult task. Still, it can be more manageable by asking the right questions and looking for specific features in a long-term care facility. It is essential to understand the individual’s requirements before deciding where to place them in residential care, especially in light of the numerous possibilities available. Some of the key points should be remembered in your mind before choosing care homes,i.e.,

  • Take some time to consider what you want.
  • Consult with your friends and relatives.
  • Make many phone calls to different senior care facilities.
  • Pay a visit to the facilities.
  • During your stay, feel free to ask questions.
  • Return to the facility for a second time.
  • Read your contract very carefully.

ADVANTAGES OF RESIDENTIAL CARE SETTING:

Some of the advantages which can avail from residential care, are given below:

  • Social life can be improved by living in an elderly care facility. Activity programs are common in most residential aged care facilities. The list may go on and on. Social bus excursions to stores, theatres, and cafés are common.
  • Private or communal suites and bathrooms are options for residential care. Beds, nightstands, armchairs, and amenities are all included in your care suite. Others, like a television, may be provided by some elderly care facilities. You may customize your space with photographs, artworks, and souvenirs, for example. A favorite chair or computer may be brought from home depending on the room’s size.

CONCLUSION:

It may appear simpler to stay in the comfort and familiarity of your own house, and moving to a residential care setting provides you and your loved ones a lengthy list of benefits.

A residential care environment is an intentional community that you may call home, where you can get professional and personalized care in a safe and supportive atmosphere. Sometimes called “old care homes” or “nursing homes,” it is suitable for those who cannot live independently at home or need 24/7 help.
