Family Medicine Jobs: Lessons from Hawaii

If you are a family medicine locum provider, you might want to consider Hawaii for future assignments. The Aloha State has plenty of openings for practitioners of all kinds, including family medicine doctors. A recent report out of the University of Hawaii suggests that the state needs about 245 physicians right now.

Family medicine is just one area of need. The state also needs cardiologists, oncologists, obstetricians, and on and on. In relation to the University of Hawaii report, there are a couple of lessons we can learn from it. The report sheds a little bit more light on Hawaii’s ongoing doctor shortage, what is causing it, and what is being done to address it.

Physicians Leaving the State

At the center of the report is data showing that a significant number of physicians have left Hawaii last year. According to the report, 152 doctors left in 2019. Why they left and where they went to isn’t quite clear. Regardless, the loss of that many doctors was exacerbated by a few other things.

Apparently, 91 doctors retired last year as well. Another 123 reduced their work hours while four passed away. As such, the state is looking at a total loss of 137 full-time physicians along with fewer hours put in by more than 120 others.

As dire as it sounds, things are not as bad as they appear. The University of Hawaii report also shows that the number of new doctors entering practice in Hawaii has made up for the losses. And in fact, Hawaii saw a net gain of 47 doctors in 2019. Fewer than 50 barely makes a dent in the overall shortage, but at least there was a net gain.

Educating Physicians Locally

One would think that Hawaii’s tropical environment would attract doctors from all over the country. After all, if you’re looking for family medicine jobs, you might just as well look in paradise, right? Perhaps not. It turns out that Hawaii’s net gain has more to do with local education than nationwide recruiting.

The University of Hawaii says that the state used to suffer from a severe shortage of cardiologists. That changed when the John A. Burns School of Medicine added a fellowship for training heart specialists. Likewise, the Hawaii Island Family Medicine program has been concentrating on training family medicine doctors. They have helped to reduce the shortage of family medicine practitioners in the state.

Research shows that doctors trained in Hawaii are more likely to stay in the state afterward. In fact, it is not even close. Locally trained physicians are 80% more likely to practice in Hawaii compared to those trained elsewhere.

Slowly Making Progress

Assuming that what is happening in Hawaii is indicative of the rest of the country, it would appear as though we are slowly making progress toward eliminating the doctor shortage. Now we need to pinpoint where the shortages for each discipline are most profound and employ proven strategies in those places.

3 Stretches to Help Eliminate Sciatica

Back issues are more and more common these days. The modern lifestyle – especially the working life, with lots of sitting on questionable chairs in front of the screen all day, causes issues with your back. Instead of sitting all day, you should try to take breaks and go for a walk or at least do some stretching during your work day. 1) The Sitting Pigeon Pose

Sitting Pigeon is a great place to start. Can be a little bit challenging but totally worth it.

Start by sitting on the floor. Your legs are stretched in front of you and your back is straight. Legs are next to one another. Your upper and lower body should be forming a 90-degree angle.

Rest your hands on the ground. Move one leg on the knee of the other, for example, left leg on the right knee. Bend it to a 90-degree angle. Stretch your hands forward towards the toes of your right leg. If you can’t reach them, just hold in the closest position you can.

If you can reach your toes, grab them and stay in that position for at least 15 to 20 seconds. Return to the starting position and repeat with the other side of your body.

This stretch opens up the hips and releases the lower back.

2) Knee to the opposite shoulder

For this next pose, lay on the ground, both your arms and legs resting straight on the floor.

Bring your left knee up towards your chest. Bend your leg and grab it with your hands. Position your hands just below the knee.

Firstly, pull your leg towards your chest, gently but as much as you can. After that, try and pull your knee towards the opposite shoulder, for example, the left knee towards the right shoulder. Pull to comfort.

If you experience discomfort or pain, lower the amount of pressure you are exerting on your leg. Hold the position for half a minute to 40 seconds.

Return to the starting position and repeat with the other side as well. Be sure to do this stretching exercise at least 4 or 5 times for both sides of your body.

3) Standing Hamstring Stretch

We’re going to use a prop to help us out with this last pose. You will need a chair or a box.

You start by standing next to the prop. Place your heel on the chair or box and stretch the leg whos heel is resting on the box fully. The leg resting on the box or chair has to be as straight as possible.

Similar to the sitting pigeon pose, stretch your upper body towards the toes of the raised leg. Stretch as far as you can. Ideally, you should hold your toes for about half a minute.

Conclusion

Remember, do all these stretches to your own comfort. Also, many people see the best results when using stretches along with other complementary treatments – like a spinal adjustments from a skilled chiropractor.

If you start to experience pain, release the tension a bit. Lower your leg own while keeping it straight. Repeat with the other leg. Do three reps of this stretching exercise.

5 Awesome Experiences to Have in Alabama

Visiting Alabama was never something which I thought I’d do for a vacation but thenI was very kindly invited down there by my colleague Richard Radcliff Trice. Richard was born in Alabama and last Thanksgiving he asked me if I would like to go and spend time with his family down there, which I gladly accepted. Somewhat surprisingly I had a great time in the state and there are some really fun and interesting things to do. I would certainly recommend spending a few days in Alabama and when you do, here is what I suggest you do.

Fine Art

Alabama has a number of fine and contemporary art museums which house large collections of some truly impressive art exhibitions. The Birmingham Museum of art was among my favorites that I visited and they have a collection of over 25,000 artworks, truly incredible. Another museum which Richard took me to was Space One Eleven, a collection of contemporary art which features printmaking work and plenty of work by local artists. There is a really vibrant art scene in Alabama and I really enjoyed exploring it.

Neversink Pit

The Neversink pit is a something between a sink hole and a cave, which goes 16 stories down into the earth below. This is a great place to spend an afternoon and if you are feeling up for it you can rappel down the hole and witness the incredible waterfalls and nature which you’ll see on the way down. A great experience and something really fascinating to see.

U.S. Space and Rocket Center

The Space and Rocket center has the largest collection of rockets anywhere in the world and it makes for a truly great day out for anyone who loves space and space travel. There are hundreds of pieces of space memorabilia to be found here and I found it to be educational and interesting in equal measure, well worth visiting during your time in Alabama.

The Museum of Wonder

There are many museums beyond just art in Alabama but my favorite of the lot is the museum of wonder, a very weird place which is perfect for anyone who loves folk art and odd things. The museum was set up by a former taxidermist, which may give you some insight into what you’ll find here. Animal skeletons, old maps, weird things in bottles, strange roadsigns and movie memorabilia are just some examples of the strange things which you can find in the bizarre museum.

Fort Morgan

This battle post was built back in 1819 and today you can visit this place to learn about the important role which it played during the civil war and other battles which took place. A severing trip which is a great option for any history buffs who are looking to visit a site which was used in many battles.

These are just 5 of the coolest things which I did during my time in Alabama but there are a great many more for you to choose from as well.
